| Biliary stone extraction in acute pancreatitis Stone extraction via ERCP: The management of gallstone pancreatitis may include urgent sphinterotomy and stone removal. The upper panel demonstrates a prominent papillae with a visible opening. The findings suggest that a stone may be lodged in the ampulla; this was confirmed by the injection of contrast. After demonstrating the common duct stone, a papillotome was introduced into the common duct and papillotomy was performed. The papilla with a vertical incision and surrounding coagulated ampullary tissue is seen in the lower panel. A balloon extraction catheter was placed about the stone and it was extracted. The irregularly shaped 0.5 cm stone is shown to the right of the papilla. The criteria for urgent ERCP and papillotomy have not been firmly established. However, patients with cholangitis, especially with severe pancreatitis, are candidates. The role for the procedure is unclear in severe pancreatitis without evidence of cholangitis (see following slide). A movie showing this stone extraction is available on this CD. スライド271 急性膵炎における胆石排石。 ERCPによる胆石排石: 胆石性膵炎の治療には緊急の乳頭切開術と排石術が含まれる。上図では、乳頭腫大と開口所見が認められる。これらは結石が乳頭膨大部内に存在することを示唆する所見であり、造影により確定できる。総胆管結石を確認した後、パピロトームを総胆管内に挿入し、乳頭切開術を施行した。下図では、乳頭の縦切開と凝固した乳頭部周囲組織が認められる。バルーン排石カテーテルを用いて排石した。乳頭の右側に0.5cmの不整な結石がみられる。緊急ERCPと乳頭切開術の適応については未だ基準は確立していない。しかしながら、胆管炎を伴う患者、特に重症急性膵炎を伴う症例は適応の候補である。胆管炎の徴候を認めない重症膵炎症例に対するこの手技の有用性については明らかになっていない(以下のスライド参照)。 このCDでは排石術の動画をみることができる。 |
